Pfizer Pediatric Vaccination Clinic
Alpine ISD will be hosting a pediatric vaccination clinic in partnership with the Texas Department of State Health Services. DSHS will be onsite from noon till 4 PM on Friday, December 10th at the Alpine ISD administration building. They will be bringing enough Pfizer doses to vaccinate at least 40 kids that are age 5-11.
The consent form and the intake form are attached. Parents should have these forms filled out prior to the event. This will help a great deal in eliminating wait times and speeding up the event.
